Use of Population Care is benefitting more than 77,000 patients covered by value-based contracts across the 500-provider Physician Hospital Organization in Northern Michigan by streamlining care processes, monitoring quality, and helping providers identify and close gaps in care for improved patient outcomes.<\/p>\n<p>\u201cWe saw it as inevitable that our payer partners would ask us to start participating in contracts with downside risk.
