This proved to be an effective method to identify and close care gaps and improve patient outcomes, as evidenced by our strong program results.\u201d<\/p>\n
For more than 20 years, COSEHC has worked to reduce cardiovascular disease, diabetes morbidity and mortality. COSEHC\u2019s experience enabled it to be one of 29 groups awarded with a cooperative agreement under HHS\u2019 new Transforming Clinical Practice Initiative. CMS funding supports the PTN\u2019s provision of clinical expertise and technical assistance to providers throughout the Southeast.<\/p>\n
